The Opportunity:
Town Square Health is seeking a Manager, Population Health to support the execution of high-priority population health, operations, and technology initiatives. This role is ideal for a highly organized, execution-oriented professional who can take loosely defined problems and drive them to clear, measurable outcomes.
Key Responsibilities
Program & Project Ownership
- Own cross-functional initiatives from scoping through execution
- Build workplans, set timelines, assign owners, and drive accountability
- Provide regular status updates to leadership on progress, risks, and decisions
Central Operations & Automation
- Support automation initiatives across call center, referrals, medical records, billing, and medication workflows
- Identify and prioritize automation opportunities based on impact and complexity
- Partner with Technology, Operations, and Finance on build vs. buy evaluations and ROI analysis
- Help define and track operational KPIs (e.g., turnaround time, error rates, cost per transaction)
Virtual Specialist Go-Live
- Serve as the point person for identifying, vetting, and standing up virtual specialist partners to support patient care delivery.
- Assess Town Square Health’s needs for virtual specialty care—including clinical use cases, patient populations, specialty coverage, and technology requirements—and recommend a prioritized shortlist of specialist vendors.
- Partner closely with the VP, Provider Strategy & Population Health to translate strategic vision and priorities for virtual specialty care into executable plans, while owning day-to-day management of specialist relationships.
- Own the specialist partner relationship end-to-end, including contract coordination, technical and operational go-live, and post-launch stabilization once patient care is live.
- Act as the internal owner and connector for virtual specialist partnerships, working closely with clinical, technology, and operations leaders to inform decisions around workflows, clinical hand-offs, technology integrations, and performance expectations.
Population Health Build
- Conduct structured vendor evaluations focused on ROI and operational impact
- Support vendor selection and implementation planning
- Help design population health workflows and operational handoffs
- Support the chronic condition management program builds
Qualifications
- 3–6 years of experience in healthcare operations, value-based care, healthcare technology, or consulting
- Demonstrated ability to manage projects end-to-end with minimal oversight
- Strong organizational, analytical, and problem-solving skills
- Ability to operate effectively in ambiguous, fast-paced environments
- Excellent communication and stakeholder management skills
- Experience working cross-functionally with clinical, operations, and technology teams
