We are opening our first multi-specialty health centers serving Medicare-eligible patients, with plans for expansion into multiple markets.

The Opportunity
We’re seeking a Manager of Outreach and Education to lead Town Square Health’s community outreach and education efforts across 3–5 Town Square Health centers. This mid-level management role is responsible for developing and executing outreach strategies that drive patient acquisition, engagement, and retention, while strengthening Town Square Health’s presence within local communities.
This role supervises Patient Engagement Coordinators and works closely with health center leaders, marketing, and clinical teams to ensure outreach activities are aligned with organizational goals and community needs. The position must be located near Rochester, NY and will maintain a hybrid work environment, with frequent travel to health centers and community-based events.
Key Responsibilities
Community Outreach & Partnerships
- Develop and execute outreach and education strategies to support patient growth, engagement, and retention goals.
- Build and manage relationships with payer sales teams and independent insurance brokers.
- Build and manage relationships with community-based organizations, employers, nonprofits, faith-based organizations, and local leaders.
- Use HubSpot CRM to manage outreach leads.
- Identify and oversee community partnerships and sponsorship opportunities that enhance Town Square Health’s visibility and trust.
- Serve as a key ambassador for Town Square Health in local communities.
Events & Education
- Plan, coordinate, and oversee health fairs, community events, and educational programs focused on patient acquisition and health education.
- Ensure events are culturally responsive, accessible, and aligned with community needs.
- Track event outcomes and continuously optimize outreach approaches based on performance data.
Team Leadership & Management
- Supervise and mentor a growing team of up to ten Patient Engagement Coordinator across 3-5 health centers, fostering collaboration, creativity, and accountability.
- Develop clear performance goals and ensure alignment across all internal and external resources.
- Ensure consistent execution of outreach best practices across markets while allowing for local customization.
Patient Engagement & Retention
- Partner with clinical, operations, and marketing teams to support patient outreach, onboarding, and retention initiatives.
- Monitor and report on outreach, engagement, and retention metrics.
- Use data and community insights to refine outreach strategies and improve patient experience.
Collaboration & Reporting
- Collaborate cross-functionally to align outreach activities with broader organizational initiatives.
- Provide regular updates and insights to the Vice President of Community and Patient Engagement.
Qualifications
- Bachelor’s degree or equivalent experience required.
- 5–7 years of experience in community outreach, community engagement, patient engagement, or related roles; healthcare experience strongly preferred.
- Experience working with payer sales teams and insurance brokers strongly preferred.
- Demonstrated success building community partnerships and executing outreach programs.
- Experience with using CRM or data-driven methods to track and manage leads.
- Experience planning and managing community events.
- Strong organizational, communication, and relationship-building skills.
- Comfortable working in a fast-paced, growth-oriented environment.
- Willingness to travel frequently to health centers and community events.
- Experience supervising, mentoring, and providing performance management to direct reports
- Excellent leadership, collaboration, and communication abilities.

What We Offer

- Starting salary range of $82,000-$87,000
