The Opportunity
This is not a traditional operations role.
Town Square Health is opening its first primary care center in Rochester in Fall 2026. The Regional Clinical Operations Manager is the person who makes sure the clinical operations model works in practice — not just on paper.
This is primarily an architecture role. You will design the workflows and operational standards that govern how care is delivered across our Rochester centers, and build the playbooks that scale as we expand. Day-to-day execution lives with each center’s Nurse Practice Manager; you are building what they execute against.
Your RN background is central to how this role works. It gives you the clinical grounding to design systems that hold up in a real care environment — and your license will see use. This role includes direct clinical work when the center needs it, keeping you close enough to patient care that your operational decisions stay grounded in reality. You will also be Town Square Health’s primary source of frontline operations intelligence, translating what’s actually happening in the centers into input that makes central teams smarter about how they build.
You’ll have the opportunity to:
- Design the clinical operations model for our Rochester centers from the ground up
- Build the playbooks and standards that will scale as we expand into new markets
- Serve as the critical link between what’s happening at the frontline and how central teams build and improve
- Lead hiring and onboarding of future Nurse Practice Managers as we grow across the Rochester/WNY region
- Stay close to patient care through hands-on clinical work that keeps your operational decisions grounded in reality
What You’ll Do
You’ll split your focus between building the systems that govern how care is delivered and staying close enough to the ground to know when those systems need to change. This role requires both the discipline to design at scale and the humility to keep learning from what’s actually happening in the clinic.
Clinical Operations Design & Standards
- Design and maintain the clinical operations implementation model for Town Square Health’s Rochester centers: workflows, clinical protocols, scheduling standards, after-hours coverage protocols, and more
- Build and lead the continuous quality improvement infrastructure for center operations, including the measurement systems and improvement loops that keep standards high as we scale
Operational Intelligence & Central Team Partnership
- Serve as the primary source of ground-level insight for central teams — translate what’s actually happening in the centers into structured input for technology, population health, and operations
- Identify where tools, workflows, and system configurations don’t match center reality, and drive the changes needed to close those gaps
- Represent clinical operations in technology build-out including EHR configuration, product development, and facility planning
Nurse Practice Manager Hiring & Onboarding
- Lead hiring and onboarding of future Nurse Practice Managers and other staff as Town Square Health expands across the Rochester/WNY region
- Build the training and onboarding framework that sets new clinical staff up to succeed and ensures consistency across centers
Clinical Coverage
- Maintain active RN licensure and expect to use it — this role includes hands-on clinical work when the center needs it, including supporting the care team and backfilling the Nurse Practice Manager during PTO or staffing transitions
Who You Are

You can move between the details of a clinical workflow and the bigger picture of how it fits into a scalable model, and you communicate clearly across both clinical and non-clinical audiences.
You’re self-directed and comfortable owning a function with limited oversight. You don’t wait for structure to be handed to you — you build it. And you stay curious about what’s actually happening at the frontline, because that’s what makes everything else better.

Qualifications
- Active RN license required
- 3+ years in healthcare operations or clinical program design, with direct experience building systems, not just running them
- Proven ability to translate clinical priorities into operational workflows and standards
- Strong analytical communicator: comfortable synthesizing frontline observations into clear, actionable input for non-clinical audiences
- Experience working across clinical and non-clinical stakeholders
- Self-directed; able to own a function with limited oversight and build structure in ambiguous environments
Strong Pluses
- Experience in a value-based care, ACO, or capitated primary care environment
- Familiarity with Athena or comparable EHR systems
- Background in continuous quality improvement methodologies
- Experience hiring or managing Nurse Practice Managers or similar clinical staff
- Pre-launch or startup experience
