The Opportunity

This is not a traditional medical assistant or LPN role.

As a TSH Navigator, you’ll be a vital member of our care team; a trusted partner and advocate for a dedicated panel of patients throughout their healthcare journey. You’ll go beyond the standard duties of a medical assistant to build real relationships, learn each patient’s individual needs, and ensure their care is well-coordinated and genuinely personal.

You will be the primary point of contact for your patients–whether in the clinic, or between visits. Patients will count on you to answer questions, coordinate appointments, address concerns, and connect them with the right resources at the right time.

You’ll have the opportunity to:

  • Serve as a consistent, trusted guide for a panel of Medicare patients
  • Bridge clinical care and community resources to remove barriers for patients
  • Help define what seamless, proactive, patient-centered care looks like as we scale

What You’ll Do

You’ll be at the intersection of clinical excellence and patient advocacy, blending hands-on medical assistant duties with proactive care coordination. This isn’t just about rooming patients and collecting vitals–it’s about knowing your patients, anticipating their needs, and making sure nothing falls through the cracks. 

Patient Care & Clinical Support

  • Greet, check in, and room patients; update demographic, pharmacy, and billing information
  • Take and record vital signs, administer visit-appropriate screening tools, document reason for visit and current symptoms, and prepare patients for exams
  • Conduct laboratory procedures including point-of-care testing, specimen collection, and processing

Care Coordination & Navigation

  • Serve as the primary point of contact for a dedicated patient panel
  • Communicate with patients to support follow-up, answer questions, and coordinate care
  • Coordinate referrals, specialty appointments, and medical records; support patients in addressing social determinants of health

Required Qualifications

This role is open to qualified candidates credentialed as a Medical Assistant (MA) or Licensed Practical Nurse (LPN).
 

Medical Assistant (MA)

  • Graduation from an accredited medical assistant program
  • Medical assistant certification (as required by state)
  • High school diploma or equivalent
  • Phlebotomy skills
  • Current CPR or BLS certification

Licensed Practical Nurse (LPN)

  • Active, unrestricted LPN license in the state of practice
  • Graduation from an accredited practical nursing program
  • Current CPR or BLS certification
  • Phlebotomy skills 

Skills and Knowledge

  • Excellent judgment and critical thinking skills with a willingness to problem-solve
  • High emotional intelligence, integrity, and adaptability in a fast-paced healthcare environment
  • Effective communicator with the ability to engage patients across diverse backgrounds and needs
  • Familiarity with healthcare systems, community resources, and care coordination practices
  • Mission-driven, collaborative mindset and a passion for improving the patient experience
